If you have changed your name within one year of the issuance of your past passport due to marriage, or death of a spouse; you can apply for a new passport free of charge. Please see the instructions for a Passport Renewal, and include documentation for your name change (e.g. original or copy of marriage certificate, divorce decree or court order and one photocopy.)
If you change your name for any other reason, please describe your reason in an email to the appropriate Consular office in your district. The consular officer in that location will provide you with additional instructions
If the change is more than one year after issuance, please follow procedure for a Passport Renewal and include documentation for name change (e.g. original or copy of marriage certificate, divorce decree or court order and one photocopy.) You will need to pay regular Renewal fees for this service.

After you have completed and printed out the appropriate application form listed above, please schedule your appointment at the U.S. Embassy or one of the Consulates General.
