All first time applicants must appear in person at the appropriate Consular office.  The application should be filled out prior to your visit.  All applicants claiming U.S. citizenship who do not have any proof of citizenship need to follow the procedure indicated here

For Adults (age 16 and older)

You will need:

  • Certified, sealed birth certificate, Consular Report of Birth Abroad or Certificate of Naturalization and one photocopy
  • One valid form of photo identification (e.g. foreign passport, driver’s license, military ID, etc.) and one photocopy
  • One passport photograph without glasses (taken within the last six months); photos can be obtained from a photo booth located in the consular sections in Rome and Naples for 6 euros and in Milan for 5 euros
  • Completed form DS-11 Note: you cannot currently list a “Country” when completing the “Emergency Contact” section on our forms. Please list an emergency contact in the United States.
  • If you are age 16 or 17, evidence of parental awareness
  • Applicants may pick up passports at the office where the application was submitted or have it delivered. Please follow this link for information on courier services.

Children Under 16

The presence of the child and both parents are required at the time of application for children under age 16. If one parent/guardian cannot go with the child to apply for the passport, they can give permission by completing Form DS-3053 “Statement of Consent.” You must submit the completed form with the child’s passport application. The parent that cannot go with the child must:

  • Sign and date Form DS-3053 in the presence of a certified notary public, and
  • Submit a photocopy of the front and back side of the ID that they present to the notary public with Form DS-3053. Please note: In certain countries, a DS-3053 must be notarized at a U.S. embassy or consulate and cannot be notarized by a local notary public.

For your child’s application, you will need:

After you have completed and printed out the appropriate application form (DS-11) please schedule your appointment at the U.S. Embassy or one of the Consulates General.
